Welcome: the job that looks simple
Burjisiya Industrial Services, a maintenance contractor in Zubair, Basra, has won a contract to clean four crude oil tanks and to repair pumps in two sewage wet wells. Every one of these jobs means sending people into a confined space.
The operations manager, Mr. Tahseen, has asked the HSE engineer, Eng. Rafid, to make sure that every entry is planned, tested and supervised, and that nobody ever goes in without a rescue plan. You will follow Rafid's team through this course: Shahad, the authorised gas tester; Yaseen, the attendant; and the entrants Saif and Bashar.
Why confined spaces kill

Confined spaces are not rare or exotic. In the United States alone, more than a thousand workers died in them in eight years, and the number did not fall. Iraq does not publish similar figures, but the same tanks, sewers, pits and vessels are found on every oilfield, refinery, port and water plant.
The danger is that a confined space usually looks harmless: an empty tank, a quiet manhole. The hazard is often invisible, and it acts in seconds.

How the workers died

Look at the gold bars. Breathing a harmful substance and oxygen-deficient air together killed 165 people. Hydrogen sulphide (H₂S) and carbon monoxide (CO) were the most common gases. Both are found in Iraqi oil and sewage work every day.

The rescuer trap

When someone collapses inside, the natural reaction is to jump in and help. A US government study found that about 60% of the people who died in confined spaces were would-be rescuers: colleagues, supervisors, even family members who went in without breathing apparatus.
The courageous act is to stay outside, raise the alarm and use the rescue equipment that was set up before the entry. This course will show you how.

Six golden rules

Every lesson in this course explains one or more of these rules in detail. If you remember nothing else, remember these six.

The rules behind this course

Iraq does not yet have a detailed confined space regulation, so oil companies and their contractors apply international standards through their permit-to-work systems. This course follows the US and UK rules, which are the basis of most company procedures, and points out where companies are stricter.

Check your understanding
A worker in a valve pit stops answering and is lying still. What should his colleague at the top do first?
About 60% of confined space deaths are would-be rescuers. Entering without breathing apparatus usually creates a second victim.
Why can you not rely on smell to detect hydrogen sulphide?
H₂S smells at low levels, but high levels paralyse the sense of smell, so the warning disappears when the danger is greatest.
